Confidentiality

Dr. Kulbatski’s assessment and therapeutic services are strictly confidential. Any information shared with Dr. Kulbatski is done so in the strictest of confidence. Information about a client is never released to anyone without written permission from the client, except as required by law in Ontario.

Please note that exceptions to confidentiality include:

  • Informing an appropriate health care professional or the police of a client’s intent to end his/her life or someone else.

  • Informing the Children’s Aid Society if it is discovered that a child is at risk or is being sexually abused, physically abused, or neglected.

  • Reporting a health professional who has abused a client or patient.

  • Informing a potential victim of violence of the client’s intention in inflict harm.

  • Releasing a client’s file if it is subpoenaed by a court of law.

In order to maintain their confidentiality in a public place, Dr. Kulbatski will not approach or greet an existing or past client. As most psychologists do in such cases, Dr. Kulbatski will only greet a client if she is approached first.


Referral Sources

Dr. Kulbatski services do not require a referral from a family doctor. However, various professionals often referring to Dr. Kulbatski’s practice include: family physicians, psychiatrics, chiropractors, lawyers, psychologists, health management consultants, occupational therapists, massage therapists, etc.


Fees

Psychological services, although not covered under OHIP, are covered by most Extended Health Care Plans or Private Health Care. Patents may also be eligible through other third party coverage, including accident benefits and disability benefits.

In cases where clients have limited or no coverage, short-term therapy could be considered as an option. Short-term therapy can often be completed within 3-5 sessions. Frequently, just several sessions can be instrumental in helping clients learn about themselves and their needs and how best to cope with their presenting issues.

Dr. Kulbatski’s fees are set within the Guidelines and the Fee Schedule provided by the Ontario Psychological Association. All psychological services are billed on a fee-for-service bases.
